Health Services reminder

If you need access to any school-related medical forms and/or action plans for your child, this is the page to access health information and forms.
 
If your child suffers from asthma, diabetes, seizure disorder, food allergy and/or an insect allergy, you must complete an Action Plan. Your family physician should also sign the completed form and return it to your child’s school. 
 
If your child needs to take prescription medication or over the counter medication while at school, a Medication Authorization must be completed by the prescribing physician. Medication cannot be administered without the medication authorization form on file.
 
The State of Illinois requires certain health requirements for students new to the district and for those at the grade levels listed below. Health records must be submitted by October 15th.  
 
Preschool: School physical, up-to-date immunization record, and lead screen
Kindergarten: School physical, up-to-date immunization record, lead screen, eye exam, and dental exam
Second Grade: Dental exam
Sixth Grade: School physical (a sports physical does NOT meet requirement), up-to-date immunization record, and dental exam
Ninth Grade: School physical (a sports physical does NOT meet requirement) and dental exam
Twelfth Grade: 2nd dose of meningitis vaccine
 
Any out-of-state students attending Illinois schools for the first time must have an Illinois physical exam, an eye exam, and proof of up-to-date immunizations prior to enrolling.
